  2. Post the form on or near the laboratory door.

  1. Fill out a Hazardous Materials Disposal Request Form
  2. Send the form to Leldon King by fax at 575-6474 or by mail at EH&S, PHPL 175.

• To dispose of Radioactive Waste:
